Your server and your client files do not match. Make sure your server is running from an exact copy of the client files you are playing with (if you have client version 7.12.1, your server should be using those same files as well). The other issue could be related to custom client files (like if you downloaded someone else's client files for their server). Make sure you're using vanilla client files for your playing account and your server.
