Infinity health bug

On our server sometime when players get hit by monster it will give players infinity health, and they will be able to do anything in game with invincibility.

Wonder if anybody know this bug, or why it's happening we've reset the server, but it keep giving players invincibility.
 
Might want more information or investigation to start somewhere.
The source could be anything.

-Do you have custom scripts?
-Did you make any recent changes?
-Have you updated recently and from which version?
-The server is running which era?
